Your envelope containing this completed award form, your store receipt with purchase dated no later than January 31, 2006, the UPC/barcode cut from qualifying product purchased, and completed registration form must be received no later than March 31, 2006. Any claims received after applicable dates will be ineligible. Faxes will not be accepted.
